• Application for Professionals (Contract Employees ONLY and if not already submitted)
• Criminal History Record Information Addendum (Included with Application for Professionals)
• Pre-Employment Affidavit
• Employment Offered Affidavit (Notary required)
• Authorization to View Employee's Examination Tab on Educator Certification Online System (TEA Certified Employees ONLY)
• W4 (Used to compute the amount of federal income tax to deduct and withhold from the employee's pay)
• I-9 (Verification of identity and employment authorization of individuals hired for employment in the United States.)
  *Copy of Driver License (must not be expired)
  *Copy of Social Security Card
• Direct Deposit Authorization
• Form SSA-1945 (Statement Concerning Your Employment in a Job Not Covered by Social Security)
  *College Transcript - Professionals (Certified transcript required)
  *Non-contract Employees/ High School Transcript or GED Certificate
• Notice of Worker's Comp Requirements (Read only)
• Acknowledgment of Receipt of Notice of Worker's Comp Requirements
